Parent Permission Form

Please fill out and submit the Parent Permission Form before the Parent Information meeting. Hard copies of the form will be available for review and signature at the meeting; if you do not submit it before the parent meeting, or if your parent/guardian is not present at the meeting, he/she must come in to the YES Company offices to review and sign the form. Please call 238-6850 to schedule an appointment.

Contract of Commitment Form

Please fill out and submit the Contract of Commitment Form before the Parent Information meeting. Hard copies of the form will be available for review and signature at the meeting; if you do not submit it before the parent meeting, or if your parent/guardian is not present at the meeting, he/she must come in to the YES Company offices to review and sign the form. Please call 238-6850 to schedule an appointment.

Report a Conflict

ALL CONFLICTS MUST BE SUBMITTED AT LEAST 24 HOURS IN ADVANCE. If you have a tentative or confirmed conflict during the span of rehearsal (general Monday-Friday 9:00 am to 5:00 pm rehearsal, or 10:00 am to 10:00 pm for production week) you must send in your conflict. Please note that filling out this form does not make the conflict an excused absence, and that all absences, excused or not, can result in recasting/redistribution of responsibilities as needed. YOU WILL ONLY BE CONTACTED IF THE CONFLICT IS NOT APPROVED.
