YES Kidettes - Forms
Thank you to everyone who completed the Parent Permission Form online
by the deadline September 27, 2011. If you did not, hard copies of the form
were sent out with each student for you to fill out and sign. The forms
are due Wednesday, September 28th. If you still have not filled out the
form, you must come in to the YES Company offices to fill out and sign the form. Please call 238-6850 to schedule an appointment.
ALL CONFLICTS MUST BE SUBMITTED AT
LEAST 24 HOURS IN ADVANCE. If you have a tentative or confirmed conflict during the span of rehearsal (general
Tuesday-Friday 4:00 am to 6:30 pm, Saturdays 9:00 am to 12:00 pm rehearsal, or
3:00 pm to 10:00 pm for production week) you must send in your conflict. Please note that filling out this form does not make the conflict an excused absence, and that all absences, excused or not, can result in recasting/redistribution of responsibilities as needed.
YOU WILL ONLY BE CONTACTED IF THE
CONFLICT IS NOT APPROVED.
